Mr. Kostuchenko is a co-founder and Managing Partner of EEF. Prior to EEF, Mr. Kostuchenko was a member of MTS Health Partners, LP (“MTS”) where he focused on growth equity investments and lower-middle market buyouts. Prior to MTS, Mr. Kostuchenko was a member of KPS Kapital, LLC, a boutique merchant bank, and prior to KPS, he worked in the investment banking division at Salomon Brothers. Mr. Kostuchenko serves on EEF’s Investment Committee. He serves on the Board of Directors of NextCare Urgent Care, Prospira PainCare, STARUS Medical Group, Triton Technologies, VantagePoint, West Dermatology and WhiteGlove Health. Mr. Kostuchenko received a Bachelor of Arts from Brown University.
Mr. Pegg is a Partner at EEF. Prior to EEF, Mr. Pegg spent ten years at MVP Capital Partners. While at MVP, Mr. Pegg focused on lower-middle market buyouts and growth equity investments in healthcare, insurance services, building products and rotorcraft services. Before joining MVP, Mr. Pegg was an Analyst for J.P. Morgan Chase Private Bank. Mr. Pegg serves on the Board of Directors of MedTest and Prospira PainCare. Mr. Pegg holds a B.A. from Yale University and an M.B.A. from Villanova University School of Business.
Group President of Operations for United Surgical Partners International, Inc.
Mr. Garvin, 51, serves as Group President of Operations for United Surgical Partners International, Inc. He joined USPI in 2001 through the acquisition of OrthoLink Physicians Corporation where, in 1996, he was a founding member of the senior management team and had served in various senior level operations and development positions, including SVP of Operations & Development and COO. While at USPI, he has participated in senior operational roles including SVP Operations, COO and currently Group President of Operations. From 1991 to 1996, Mr. Garvin was Vice President of Operations at Surgical Care Affiliates/HealthSouth. He began his healthcare career as a Financial Analyst for a small healthcare company in Nashville, TN after completing his BBA in Marketing at the University of Kentucky and his MBA at the University of South Carolina.
Douglas W. Lehrman
Founder and Chief Executive Officer, Pulse Equity Partners, LLC
Doug is the Founder and CEO of Pulse Equity Partners. Pulse is a focused growth equity investor in healthcare services, active lifestyles and wellness. Doug has been an investor in this area for over fifteen years. Doug is currently on the Board of Prospira PainCare, In Shape Health Clubs, a 71 unit company owned chain in California, and PhysicianOne Urgent Care, a 10 unit urgent care business currently operating in Connecticut. Doug is also on the Board of a not for profit, Project 375 focused on eradicating the stigma in mental illness. In addition to his 15 plus years of private equity experience, Doug was an investment banker previously. He received a BA in Economics from Washington University in St. Louis and an MBA from the Stern School of Business at New York University.
David P. Malm
David Malm is the Co-Managing Partner of Webster Capital and leads Webster’s healthcare investment practice. David Joined Webster Capital in 2007 concurrent with the launch of Webster Fund II ($200M) and has led all of Webster’s healthcare investments including Comfort Keepers, Epic Health Services, Freedom Home Health, Discovery Practice Management, Prospira Pain Care, Conisus, Loving Care Agency, PharmaLogic, HQRC and BAART Methadone Programs. Prior to joining Webster Capital, David spent 15 years at Halpern Denny & Company (led by the co-founder of Bain & Company) where he led healthcare investments in cardiac diagnostic imaging, radiation oncology, hospice, physical therapy, and sleep therapy. He also led investments in the consumer and media sectors including such companies as Barneys New York, Nextwave Telecom, Picturetel, Hollywood Digital and Todd-AO.
David began his private equity career in 1987 at Bain Capital. Prior to Bain Capital, David worked in strategy consulting at Bain & Company and also worked in the M&A investment banking practice at Morgan Stanley & Company. He has a BA from Brown University and an MBA from Harvard University.
Executive Chairman, Prospira PainCare
Kevin Miller serves as Executive Chairman for Prospira Paincare’s Board of Directors and has been with Prospira since 2014. Prior to joining Prospira, Kevin served as Chairman and Chief Executive Officer of Benevis, a national multi-unit dental practice; Vice President and General Manager at Ariba, Inc. and Principal with The Parthenon Group. He began his career in GE’s Financial Management Program and later worked at GE Capital as part of a global M&A team. Kevin received his MBA from Dartmouth’s Tuck School of Business and a bachelor’s degree in Business Administration from the University of Massachusetts at Amherst.

Bill Brigham serves as President and Chief Executive Officer of Prospira Paincare since August 2014. Prior to joining Prospira, Bill served as President of Hospitalist Medicine for ApolloMD. Previous to this, he served as Chief Operating Officer of Benevis, a large multi-unit dental practice; and also held various leadership positions within large retail corporations.

The Pain Management Center
Dr. Adam Sackstein is the founder and medical director of The Pain Management Center, a Prospira PainCare Center of Excellence. Dr. Sackstein received his undergraduate degree from City University of New York, Brooklyn College and his medical degree from State University of New York Health Science Center of Brooklyn College of Medicine after completing a seven year combined BA/MD program. Dr. Sackstein’s internal medicine internship was completed at Staten Island University Hospital and his anesthesiology residency and pain medicine training was completed at Yale University-New Haven Hospital. Dr. Sackstein is board certified in pain medicine and anesthesiology and is a diplomate of the American Board of Pain Medicine. He is also a past President of the New Jersey Society of Interventional Pain Physicians and continues to serve on its Executive Board. Dr. Sackstein is also Chairman of the Board of the Hamilton Surgical Center located in Hamilton, New Jersey and Treasurer of Premier Surgical Center located in Marlton, New Jersey.
